Interdisciplinary teams are critical components of care provision and improve quality care delivery as they focus on one common goal in patient treatment plan. As such, respect and professionalism are core components of interdisciplinary team (Stanley et al., 2022). Members of interdisciplinary teams have a duty to ensure that patient records and information is accurate to avoid adverse effects and medical negligence instances. The core ethical principle in the case scenario is veracity or the obligation to tell the truth and not deceive (Ralph et al., 2019)). The principle is critical in healthcare as it promotes a culture of trust, respect, and integrity among stakeholders; especially patients and providers.
In this case scenario, I will approach the physician privately and express my concern about the discrepancy in observation based on the information in the patient’s chart and the observed or behavior. I will communicate in a respectful and professional manner without any confrontation while focusing on the overall wellbeing of the patient. For instance, I would initiate a dialogue with the physician.
“Excuse me, doctor. I noticed that in the patient’s chart, you mentioned that they are alert and oriented three times. However, during rounds, we observed the patient presents signs of confusion. Could we review the information to ensure the chart’s data is accurate?
Organizational culture plays a pivotal role in the management of errors in any healthcare setting. For instance, when a safety culture exists, healthcare providers like nurses communicate openly and clearly, learn from mistakes and seek continuous improvement. Promoting open and effective communication encourages respect and understanding of team roles and responsibilities. Learning from mistakes ensures that the company does not entrench a culture of blaming others for errors (Yoder-Wise et al., 2022). Such a culture focuses on the root cause of errors and implements systematic changes to prevent future occurrences. Continuous improvement ascertains that providers focus on better patient outcomes and seek ways to improve processes through lifelong learning and implementation of evidence-based practice (EBP). Conclusive ethical principles are important in healthcare as they help improve overall care delivery and teamwork.
