The PMHNP should first assess the client’s current symptoms, including his rationale for
not taking his medications. It is important for the PMHNP to listen to the client’s
concerns and be understanding of his struggles with medication side effects. The PMHNP
should explain to Mark that although the medications may have caused uncomfortable
side effects, medication compliance is necessary to help him manage his schizophrenia.
In addition, client should be educated that alcohol misuse alone over long periods can
cause psychotic symptoms such as hallucinations. Which are positive symptoms that
clinicians try to eliminate or diminish with medication in schizophrenic patients. The
PMHNP should also explain the risks associated with not taking medications, such as
increased likelihood of relapse, as well as the potential benefits of adherence. It is also
important to discuss strategies to manage side effects, such as adjusting the dose or
switching medications (Bright, 2017)
